30-DAY SATISFACTION GUARANTEE

If you are unsatisfied with your Intake or purchase for any reason, you have thirty (30) days from the date of purchase to request a refund.

To qualify for a replacement or refund, all the following conditions must be met:

  1. A Return Merchandise Authorization (RMA) must be requested from our customer service team within thirty (30) days of your purchase date. To request an RMA, contact our support team.
  2. Product must verify the order number of your purchase for the date of purchase. The order number can be found in the following locations:
  • On the order confirmation email sent upon completion of checkout.
  • On the shipping confirmation email sent upon shipping fulfillment.
  • On the packing slip included with the order upon shipment.
  • If none of the above are available, please get in touch with customer support.
  • Once a purchase has been submitted for an RMA request, that product will no longer qualify for any other guarantee or warranty.
  • Returned Product must be in good physical condition (not physically broken or damaged) and in original packaging.
  • All accessories originally included with your Product must be included with your return.
  • If you receive a refund, the cost of return shipping may be deducted from your refund
