30-DAY SATISFACTION GUARANTEE
If you are unsatisfied with your Intake or purchase for any reason, you have thirty (30) days from the date of purchase to request a refund.
To qualify for a replacement or refund, all the following conditions must be met:
- A Return Merchandise Authorization (RMA) must be requested from our customer service team within thirty (30) days of your purchase date. To request an RMA, contact our support team.
- Product must verify the order number of your purchase for the date of purchase. The order number can be found in the following locations:
- On the order confirmation email sent upon completion of checkout.
- On the shipping confirmation email sent upon shipping fulfillment.
- On the packing slip included with the order upon shipment.
- If none of the above are available, please get in touch with customer support.
- Once a purchase has been submitted for an RMA request, that product will no longer qualify for any other guarantee or warranty.
- Returned Product must be in good physical condition (not physically broken or damaged) and in original packaging.
- All accessories originally included with your Product must be included with your return.
- If you receive a refund, the cost of return shipping may be deducted from your refund
